Geriatric Healthcare Consultants, LLC (GHC) is seeking both full and part-time Social Service Consultants to provide consulting support to facilities in California, Oklahoma, and North Carolina.We are an exciting team of Social Workers, Therapeutic Recreation Specialists, Behavior Specialists and Dementia Care Specialists dedicated to improving the quality of life for residents in skilled nursing facilities.GHC provides consultation to over 500 skilled nursing facilities throughout the continental United States as well as the Hawaiian Islands.
Successful candidates will possess the knowledge, skills and abilities necessary to:
•Consult with Social Workers and members of the interdisciplinary
treatment team within skilled nursing facilities.
• Provide guidance to dementia care providers – especially dementia of
the Alzheimer’s type.
•Review and make recommendations concerning Medical Records
documentation and Psychosocial assessment.
•Provide guidance to Social Workers and members of the
interdisciplinary treatment team on topics concerning the medically
related psychosocialneeds of residents.
•Provide guidance to facility Social Workers concerning discharge
planning.
In addition, the candidate must:
•Be familiar with federal OBRA regulations.
•Possess an LCSW and/or MSW from an accredited school of Social
Work.Graduate students enrolled in an MSW program will be
considered for the position.
•Be willing to travel 2-4 days per week.
